Send the Documents. - The next step once you have accumulated all the necessary forms is to send them to the Hawaii Lieutenant Governor's office located at: Office of the Lieutenant Governor, State Capitol, South Beretania Street, 5th Floor Honolulu, Hawaii 96813 Publication to Newspaper. - After filing, the lieutenant governor's office will send back to you the Notice of Name Change. You will then send the Notice to any newspaper that serves your area. All you need is to publish in the newspaper for 1 day, and then the newspaper will send back to you the publication affidavit. Send Affidavit. - Upon receiving the publication affidavit, you will be required to send the original affidavit back to the lieutenant governor's office at the address previous stated above.
