PaJR -- Patient Journey Record -- Reducing Avoidable Hospitalisations The PaJR (Patient Journey Record) system helps identify patients at most risk of avoidable hospitalisation or re-admission after discharge from hospital through a personalised care management approach for patients combined with cutting edge machine learning analytics. Current estimates for the bill for avoidable re-admissions in the US is $15 billion with a corresponding figure of £1.5 billion published by the UK Department of Health in 2010. From 2013, US hospitals will be financially penalised for every re-admission of a Medicare patient within 30 days of their discharge. The number of patients over 65 years of age admitted to hospital in the US is 13 million per year. Currently, 20% of these patients are re-admitted within 30 days. Initial trials of PaJR's Re-Admission Prevention (RAP) Service have shown a 50% decrease in re-admissions. The PaJR RAP Service is a cloud based system used to track patient and/or caregiver self-reported health status using an innovative clinical workflow that conforms with data protection standards. The proprietary PaJR Machine Learning language engine analyses the data recorded and in real time flags alerts in key indicators that trigger appropriate interventions with the patient when necessary. The PaJR RAP service can achieve greater success in reducing avoidable hospitalisations than connected health devices offered by Intel/GE, Phillips, Bosch and others. These current market leaders monitor and generate alerts based on biometric signals such as blood pressure or oxymeter readings that require devices in the home and generally employ expensive nurse specialists using disease specific triage processes to interpret the data, while PaJR through a simple phone call can generate similar alerts. The PaJR solutions for monitoring patients recently discharged from hospital and for patients identified to be at risk of hospitalisation is up to 10 times cheaper than current care management and hardware monitor devices.
